Advice on questions regarding the need for care
In Weiterstadt, a community nurse provides easily accessible advice to people who are at risk of needing care or who already have a care need. The community nurse, a qualified nurse with the German Red Cross, works locally and offers advice in people's homes when daily life becomes more difficult and it is unclear what kind of support is appropriate.
The focus is on a confidential conversation. Together, the current situation is assessed and possible next steps are discussed. The community nurse provides information about support services in the district and, if needed, establishes contacts with appropriate resources. The consultation takes place only with the consent of the person concerned and is tailored to their individual wishes.
Support when family support is lacking
When family caregivers live far away or no family support is available, organizing home care often becomes more difficult. The community care worker cannot replace missing family members, but she can help to realistically assess the situation and identify suitable ways to provide care at home.
